Local Governments and others: Submit agency order form or use the State of Illinois standard Order for Delivery Form-IL 401-0105. (Additional forms available from Illinois Correctional Industries Springfield Marketing Office.)
By Phone: Orders may also be placed by calling (800) 634-0213 or (217) 558-2200 Ext. 7005 when an item is required immediately. Orders may be faxed to Industries at (217) 522-9642.
By E-Mail: Questions, Comments, and Completed IL401-0105 forms may be submitted via e-mail to ici@doc.illinois.gov.
CUSTOM ORDERS
Orders for special items or help with measurements on clothing, furniture, signs, silk-screen items (embroidery layouts) etc., are available by calling: (217) 558-2200 Ext. 7005 or (800) 634-0213. If you need special assistance, an Illinois Correctional Industries Marketing Representative will call or visit with you at your convenience, to discuss any details or questions. For custom signs or silk-screen/embroidery layout, any photo ready artwork will help expedite your orders. If you would like our Graphic Art Department to assist you for a nominal fee, please contact our office.
